Before Your Visit

Before you will see Dr Hardwick we have some brief paperwork for you to fill out. For your convenience you can print the following forms from home and bring them in. Please complete a PATIENT APPLICATION, and, if you were hurt at work or in an auto accident, a WORKER’S COMP or AUTO ACCIDENT history to bring with you on your first visit.

What to Expect on Your First Visit

In this office, our goal is to develop a health care treatment rapport with you. The first part of the exam is a sit down history taking session to determine if we can help you and your injuries, aches, and pains. Secondly, you may be asked to move in all directions. We call this range of motion, or in most cases, “lack thereof”, which is one of the things we try to correct. You may also have your body parts placed into different positions and you will be asked questions like, “Does this hurt?” Of course your answers may vary from “yes”, “a little”, to “why do you think I am here?!”. These tests are called orthopedic tests, or signs, and they help determine part of what may be causing your problems. They also help to narrow down the areas to be more closely evaluated, for example by X-Ray if needed, or MRI, if medically necessary. There are other tests also, like the usual blood pressure and pulse tests. We also do reflex tests, the one with the little hammer that makes your knees and legs jump, your ankle jerk, your arm, wrists and fingers move. It’s a good sign when they do this. You may also have a device attached to you that will measure the extent of what you feel or don’t feel, a disposable pin or such. Your lungs and heart sounds may also be checked.This is an over intensification of what you may be put through, but it’s defining the better part of an examination.  And if it is discovered that this office cannot help you, you will be referred out to other doctors which can give assistance.

X-Ray

X-Ray is sometimes required to get a full evaluation of a patient. The need for X-ray is considered on a case by case basis.

Report of Findings

Once all the information gathering and examinations have been performed, the doctor will give you a report of findings and tell you his recommendations for your treatment.

Treatment

Most of our patients begin to see results from the very first adjustment. Our special gentle yet effective alignment of the body is interactive with the patient, gentle enough for kids (they love us), yet able to restore function and movement for the athlete and senior.

Our office participates in many insurance programs. We also accept auto accident liens and workers compensation cases.
